I hereby acknowledge that I am voluntarily participating in the retreat organized by Christ United Ministries Int'l (CUMI) from August 8th to 10th at Cathedral Oaks retreat center. I agree to the following terms and conditions:

1. Assumption of Risks: I understand that participating in the retreat may involve certain unforeseen contingencies. I voluntarily assume all risks associated with my participation in the retreat.
2.  Indemnification: I agree to indemnify and hold harmless CUMI from any claims, losses, damages, liabilities, or expenses (including legal fees) arising out of or related to my actions or conduct during the retreat.
3. Emergency Medical Treatment: In the event of an emergency, I authorize CUMI and its volunteers to seek and consent to any necessary medical treatment on my behalf. I understand that I am responsible for any costs associated with such medical treatment.
4. Photo/Video Release: I grant permission to CUMI to use photographs, videos, or recordings of me taken during the retreat for promotional or educational purposes. I have read and understood this liability waiver and release form, and I voluntarily agree to its terms and conditions
