I hereby acknowledge that I am voluntarily participating in the program organized by Christ United Ministries Int'l (CUMI) from October 29th to November 2nd. I agree to the following terms and conditions:

1. Assumption of Risks: I understand that participating in the summit may involve certain unforeseen contingencies. I voluntarily assume all risks associated with my participation.
2.  Indemnification: I agree to indemnify and hold harmless CUMI from any claims, losses, damages, liabilities, or expenses (including legal fees) arising out of or related to my actions or conduct during the summit.
3. Emergency Medical Treatment: In the event of an emergency, I authorize CUMI and its volunteers to seek and consent to any necessary medical treatment on my behalf. I understand that I am responsible for any costs associated with such medical treatment.
4. Photo/Video Release: I grant permission to CUMI to use photographs, videos, or recordings of me taken during the summit for promotional or educational purposes. I have read and understood this liability waiver and release form, and I voluntarily agree to its terms and conditions
